We are pleased to announce the opportunity to celebrate the roles of caregivers with the Kickin’ Parkinson’s Caregiver Award. This award aims to recognize the individuals who serve as supportive advocates for Parkinson’s Disease family members or patients in the Greater New Orleans area. We invite you to complete the Kickin’ Parkinson’s Caregiver Award Application, included below, for an individual who embodies the indispensable role of caregiver and enhances your life or the life of a loved one. The chosen recipient will be recognized at Kickin’ Parkinson’s event on September 24, 2015 as well as receive $250 and complimentary spa services from Stonecreek.
We ask that all applications be emailed to slandry@ricevoelker.com by August 15, 2015 so a recipient may be chosen. The Kickin’ Parkinson’s Award Committee looks forward to reading the applications reflecting the immense love and commitment of these caregivers and hope to see you all at our event in September.
